Forms may be completed online or printed on your computer. All completed forms must be submitted to the RMES office by mail or in person. These forms are required for all students*: Application for Admission After-School Transportation Consent to Treat Drug-Free/Computer Use/Pest Management/Photo Release *Parents must also sign a financial contract for each student.
